To protect your privacy and ensure your information is released safely, medical records are provided only after we receive the appropriate authorization and required information.
How to Request Your Records
1. Complete a Medical Records Request
Submit a completed Medical Records Release Authorization with the information needed to identify your records and where they should be sent.
2. Provide Required Information
Please include:
Your full name
Date of birth
Phone number
The records or dates of service you are requesting
The name and contact information of the person or organization receiving the records
Your signature and date
Any additional information requested on the authorization form
3. Submit Your Request
You can submit your completed request through the available office contact options.
4. Records Processing
Once your request is received and verified, our team will process the request and contact you if additional information or authorization is needed.
How Long Does It Take?
Most medical record requests are processed within [5–10 business days] after we receive a complete request.
Requests that require additional review, extensive records, or information from another facility may take additional time.
To avoid delays, please make sure your authorization form is completed fully and includes accurate contact information.
